approximately 0.7 mile of Malone/Jones Creek Branch Road. Dense Grade Aggregate is included for holes in pavement as encountered. Holes, shoulders and uneven areas will be filled with Dense Grade Aggregate at the discretion of the inspector. The Dense Grade Aggregate will be compacted with a vibrating compactor. Installation of aggregate and compaction will be considered incidental to “DGA”. Currently, the surface of the roadway has areas of broken/loose material. The contractor will be responsible for removing all loose material from the surface of the roadway. Additionally, any patch material that has been applied to the roadway must be removed by the contractor prior to resurfacing. Removal of this material will be considered incidental to “Bituminous Repair”. A “Tack Coat” will be applied prior to surfacing. 700 tons of bituminous material will be used based upon the estimate of 14’ of road width at a thickness of 2”. The project site is located on Lois Tackett Drive off Hwy 979/1426. The site is being negatively impacted by surface drainage runoff. The water enters the Tackett property at the south end from a pipe on adjacent property. The runoff periodically floods the Tackett property. To correct the problem a flat bottom gabion ditch (4 feet wide) will be constructed along a hillside on the east side of the property. Some excavation of the hill may be required for the construction of the ditch. The gabion ditch will flow into a new concrete ditch which will flow to an existing dropbox next to Hwy 979/1426.
Any excavated material will be disposed on the north side of the property on the designated waste area.
A rock core subdrain will be constructed under the waste area.
Site Preparation includes clearing and grubbing of vegetation. The construction areas shall be revegetated using agricultural limestone, fertilizer, seed, and mulch. Hay bales and silt fences shall be installed for silt and erosion control. A hoe ram bid item is included for excavation work needed for the ditches, subdrains and retaining walls.
A landscape allowance is included to replace “in kind” landscape disturbed as needed as part of normal construction efforts A utility relocation/structural removal bid item is included to move any utilities that will be required. For continued description see “Special Conditions”.

The Springfield campus of Elizabethtown Community & Technical College was completed in 2010 and is sited in the Springfield –Washington County Commerce Center off Highway 555. The building is a two-story building with a partial walk-out basement level with poured-in-place concrete retaining walls. There have been ongoing leaking problems with these walls and it has been determined after some exploratory excavation and demolition that the waterproofing and thru-wall flashing were not integrated upon initial installation. This bid package addresses these issues by excavating to expose the concrete brick ledge, removal of the brick, extension of the waterproofing and reinstallation of the brick, thru-wall flashing and accessories. The contractor shall:
• Excavate to an elevation 1’-0” minimum below brick ledge
• Remove brick to expose brick ledge and thru-wall flashing
• Extend waterproofing up vertical wall surface and behind thru-wall flashing integrating into the termination bar.
• Replace thru-wall flashing
• Reinstall brick with vents and weeps
• Replace grading to allow positive flow away from building
The scope of work is located on the north wall and west wall. The work along the west wall shall be completed first as it involved removal and replacement of a portion of concrete pavement at the building’s main entrance. Upon final grading, all disturbed

This Work includes the construction of a 2,000 sf office building at 236 Bear Track Rd., Campbellsville, KY 42718. The building is slab-on-grade with wood framed walls, vinyl clad double hung wood windows, R-19 batt insulation, shop-fabricated wood trusses, asphalt shingle roofing, and fiber cement siding. Typical floor finish to be vinyl composition tile, typical wall finish to be painted gypsum board, typical ceiling to be suspended acoustical tiles. Typical ceiling height to be 8’-0”, typical bearing wall to be 10’-1”. The HVAC system is an electric heat pump. The building will accommodate an occupancy of 21 and house the following rooms and functions: a large workroom with kitchenette, four private offices, a small conference room, men’s and women’s restrooms, mechanical room, custodial room, communications room, and reception room. The Carol Walters AML Reclamation Project involves the reclamation of AML related problems at four sites within the Paintsville and Oil Springs quadrangles. G.P.S. coordinates for each site are listed below, along with a description of the problem and proposed construction activities. Mostly, this group includes open mine portals and/or mine-related drainage issues. The Carol Walters site has a waste area within the project limits that will be utilized by all sites of this group until full. If additional area is needed, waste will be taken to one of the previously approved waste areas within the vicinity.
The Billy Howard site. The complaint at this site is drainage issues, causing an over-saturated yard. AML plans to install approximately 270’ of subdrain. Approximately, 65’ will flow SSW behind the residence and then extend another 65’ toward the road and daylight into a culvert. The other 140’ will flow NNE behind the residence and then continue toward the road. The northern subdrain will empty into an installed dropbox. Collected water will then flow into an 18” RCP (with flowable fill) under Burton Lane (replacing the 12” culvert that is currently in place). A second 18” RCP (with flowable fill) will replace the existing 15” culvert under the road at the southern end of the residence. Two 4’ flatbottom class III ditches (approximately 25’ in length) will be constructed to direct the water from each culvert down the slope into a nearby natural drain.
