The work consists of replacing the existing electric domestic hot water (DHW) boiler. All demolition of the exiting electric DHW boiler and disposal shall be included. All piping, pressure relief vents, electrical work required shall be included. New concrete housekeeping pads shall be included. Removal of the existing exterior galvanized grating, pipe column and steel support beam in the transformer well outside the boiler room and reinstallation necessary for removal of the existing boiler and installation of the new shall be included. Boiler and electrical permits with final certificates are required. All work to meet requirements of Kentucky Department of Housing, Building and Code Department including Kentucky Boiler Code and all permitting and inspections and certificates required.
An alternate price for replacement of a similar boiler in the room shall be provided as a separate cost in the Form of Proposal.

PROJECT DESCRIPTION:
The work as defined in the construction documents includes specific exterior glazing replacement to the University of Kentucky – Baily Stumbo Building located in Hazard Kentucky,
directly adjacent to the Appalachian Regional Hospital (ARH). The building was constructed in
2002 and a number of the existing insulated glazing panels have lost their seals resulting in a cloudy/fogged appearance with no insulating value. The work of this project is divided into two
parts and includes 1. Replacement of specifically identified insulated glazing panels installed in
aluminum storefront framing with new insulated glazing panels. 2. Water spray testing to
determine the location of two water leaks, one located in the East Stair Tower and one located
in the West Stair Tower. Upon determining the source of the water leaks it will be required to
remove the existing insulated glazing panels located in the curtainwall framing system and
provide new gaskets and sealants as required to rectify all leaks. Existing insulated glazing
panels to then be reinstalled. A total of (6) glazing panels to be included in the base bid pricing with a unit price identified below in the event additional glazing panels will need to be repaired
due to the water spray test results.

This Request for Proposal (RFP) is issued to solicit proposals from qualified, experienced,
financially sound, and responsible firms to install the new synthetic turf game field and drainage
system, and new indoor batting/pitching turf at the new University of Kentucky Baseball Complex in
Lexington, Kentucky. This Contractor shall provide all labor, tools, materials and services required
to fine grade the soil subgrade, construct the game field drainage system, and furnish, deliver and
install a complete outdoor synthetic turf system for a Division I NCAA baseball program (game
field), and indoor batting/pitching turf system as described herein for the University of Kentucky
Athletics Department.
The University intends to evaluate as a Base Bid Field Offer for your best synthetic turf system
currently being utilized on an MLB game field or NCAA Division 1 game field, and an Alternate Field
Offer for your most cost effective turf system currently being utilized on an MLB game field or NCAA
Division I school game field. The indoor batting/pitching turf is to be included in both the base bid
and alternate bid numbers.
The firm selected would provide all design and construction services necessary to construct the
facilities in accordance with the guidelines, standards and limitations contained in the Request for
Proposal.
• The design specified in the response must provide the reviewers of the proposal with
information on the function of each component and how components are integrated for each
of the proposed game turf systems and indoor turf system. The response must also provide
a detailed overview of the capabilities of each of the proposed systems.
• The Contractor who is chosen to install the system must provide a turn-key installation with
full functionality.
• The Contractor who is chosen must provide a single point of contact during the warranty
period for all repairs. This single point of contact will have full responsibility for ensuring
repairs are completed. After the warranty period has expired the Contractor must offer a
maintenance contract on the system with a single point-of-contact for all repairs.

This project will address the Narramore (#13324) and Anderson (#5305) drainage complaints in McRoberts community in Letcher County. A waste area has been designed at the former Potter Fork Refuse AMLRP site also previously used as the waste area for the Can Tubbs Group.
At the McRoberts site, we will install drainage controls to collect mine water from the hillsides behind the homes and direct it to the streams. Currently the water flows down the hillside surface and subsurface and creates wet backyards and sheet flows on public roadways.
No trees should be disturbed at any site. Some work within the McRoberts community will occur within the 100-yr floodplain and all appropriate permits will be obtained prior to beginning work. All sites will have proper silt control structures installed prior to disturbance of soil and these measures will be maintained and updated throughout the project.
Renovation of Patient Units 3B and 3D at the Kentucky Correctional Psychiatric Center in LaGrange, Kentucky. Scope of work includes removal of existing VCT and Vinyl Base, provide new VCT and Vinyl Base and paint all vertical surfaces.
The project is located within a secure area of the facility and has special requirements such as site access, communication, vehicle parking, construction site security, etc. These requirements are included in the contract documents.
