FY22 CMOM Infrastructure Rehabilitation Project
Description and location of Project: This Contract provides for the installation of approximately 7,000 linear feet of cured-inplace pipe in sanitary mains, ranging in diameter from 8 to 24 inches, as well as approximately 180 associated laterals to be lined from the main to the property line using cured-in-place pipe, and approximately 25 open-cut pount repairs. This project also includes replacement of approximately 65 sanitary manhole castings including chimney seals, and approximately 600 vertical feet of manhole epoxy lining. The project includes all erosion control, ground restoration and appurtenant work as described in the bid proposal. The approximately 40 project sites are located in three regions around Jefferson County, see FY22 Rehab Projects Map Series for locations. All work shall be done in accordance to MSD’s Standard Specifications.

CCK-2569-22 Good Samaritan Hospital – Emergency Generator Project – 2557.0
This project will upgrade the emergency power system in the Good Samaritan Hospital facility
to a level required to properly support the current load requirements. The project will add a
new interior 750 kilo-volt-ampere generator and connect in parallel with the existing two. In
this configuration, two generators will support the facility. The third generator will be
redundant and allow for routine maintenance without compromising the emergency operation
of the hospital. The resulting design capacity will be 1800 ampere at 480 volts. This additional
generator will be placed in the existing paint shop and two adjacent storage areas.

RFB-31-22 Buechel RC Latrine Renovation

The project consists selective demolition and subsequent renovation of areas within the Buechel National Guard Readiness Center located in Buechel, Jefferson County, KY. Work includes demolition of existing Men’s Latrine, including removal and subsequent offsite disposal of existing architectural finishes, plaster ceiling system, ceramic tile flooring, doors, frames & hardware, glazed tile and concrete masonry partitions required to accommodate new building improvements. In addition work includes cutting of new openings to accommodate installation of a new Lactation Room in an existing storage space between Office 110 and the Women’s Latrine.
New work to be performed includes installation of new Men’s Latrine including fixtures and finishes, erection of new light gage metal stud and gypsum board partitions, new hollow metal doors and frames, new door hardware, new ceramic tile, solid surfacing shower enclosures, new acoustic tile ceilings, new carpet tile, MMA flooring, and limited plastic laminate casework. Mechanical improvements include new plumbing and piping, removal and reinstallation of existing VRF cassette, installation of a new panel heater, and associated systems controls. Electrical work includes new circuitry, lighting, conduit, fixtures and trim.
In association with the cited improvements, proposed work to be performed under the contract includes the abatement of asbestos containing materials (ACM) associated with pipe insulation on active water lines. The abatement contractor will use glove bag methods to abate the identified thermal system insulation consisting of air cell and associated elbows and fittings, and will conduct air monitoring for clearance sampling prior to releasing the area to the general contractor to recommence work.
Abatement tasks to be performed include: securing approval from a Subtitle D landfill, coordination with general contractor to protect existing facilities, demolition of ceiling areas to access the ACM, abatement of the ACM using glove bag methods, air monitoring as appropriate to determine areas is clear for occupancy prior to releasing area back to the general contractor.
The Project has two (2) additive Alternates, which if accepted will become part of the work. Add Alternate No. 1 includes removal and replacement of existing light fixtures throughout the Readiness Center at locations shown. Add Alternate No. 2 includes the removal and replacement of two washbasins, one single and one double, and associated fixtures in the Women’s Latrine.

Include the life cycle replacement of the existing Fan Coil Units (FCU), three modular boilers and the chiller with a new mechanical system to balance the functionality of the selected systems with the Kentucky Army National Guard’s (KYARNG) desire to improve the energy performance of their buildings while maintaining Tenant operability and maintainability. Also, include the life cycle replacement of facility components to include: lighting, refinishing of wall surfaces, and replacing tile and carpeting throughout the building. The design is encouraged to be relevant to current architecture while avoiding design fads. Select materials to be aesthetically pleasing, easily maintained, sustainable, and cost effective.

The project involves site preparation and the construction of a single-story, 1028 square-foot pole barn building consisting of a slab-on-grade, structural wood framing with metal siding at the KCTCS Hazard Campus. The proposed site is located near the corner of Jefferson Avenue and College Avenue, and at the north end of an existing asphalt parking lot. The approximate latitude and longitude of the proposed site is 37°33’16″ N and 83°22’57″ W, respectively. Site preparation for the proposed building will involve the demolition of approximately 1,370 square feet of existing asphalt paved parking lot, minor embankment construction to support the slab-on-grade structure, and the installation of approximately 42-feet of polymer concrete trench drain, polymer concrete catch basin, and 30-feet of outlet piping. The 36’ 5-1/2″ by 28’-2″ pole barn structure will be supported on 24-inch diameter concrete piers and will include three overhead doors and one man-door.

Project Description:
This project is for the construction of a 60’-3″ long x 62-0″ wide x 10’ high concrete structure with an interior concrete pad and concrete entrance apron for the purpose of bulk storage of roadway, deicing salt.
Class “A” concrete to be reinforced with epoxy coated rebar & fiber, which shall be used throughout the shallow, spread footing and walls. The Contractor shall be required to perform all excavation, filling, grading, & compaction of sub-grade and sub-base necessary for the construction of footing, interior pad, and entrance apron.
Concrete walls will be ten (10) feet high, will be twelve (12) inches thick, will be constructed on the concrete footing, and will support a pre-engineered, membrane covered (roof) structure (PMCS) with trusses on twelve (12) on-centers. The roof structure is to be furnished and installed separately by Bulk Storage, Inc., per their current Master Agreement upon completion of the footing and walls under this contract. The (PMCS) is not a part of this Scope of Work.
Nine (9) inch thick, Class “AA” concrete modified with fiber & epoxy coated rebar, concrete slabs shall be placed inside the footing and at the entrance on the exterior of the structure.
This project is to be bid LUMP SUM. This bid shall include all labor, equipment, materials, and any incidentals necessary to complete the items of work detailed in plans and herein.
