The Elizabeth Joseph Drainage site is located at 315 Hoover Hollow, Garrett in Floyd County, KY, 41630. GPS coordinates at the site are 37° 28’ 9.13″N, 82° 48’ 10.37″W. On January 3, 2022, the culvert crossing the property originally installed as part of an OSM reclamation project failed during a heavy precipitation event damaging the citizen’s yard and impacting access to the driveway servicing the residence.
To address this concern, the failed pipe will be removed and replaced with 48” HDPE pipe with headwalls on the inlet and outlet. The material generated during excavation will be used as an alternate pipe bedding or used onsite to create positive drainage in the yard area. Class II/III stone will be used to further armor erosion prone areas near the headwalls. Revegetation of all disturbed areas will be addressed using standard residential seeding practices. The impacted concrete driveway will be replaced in kind. An allowance for removal, relocation and repair of existing utilities within the work area has been included. No material will be transported to an off-site waste area.
The cost estimate for this work also includes bid items for site preparation, hoe ram, filter fabric, and roadway stone to facilitate the construction. In total, the project area encompasses approximately 0.5 acres between the public road (Hoover Hollow) and the citizen’s residence as marked on the plans.
This project will not address underground mine openings. No trees will be disturbed outside of the unstable landslide area. All areas disturbed by this project will be revegetated by means of standard AML residential seeding practices. Sediment control will be provided by installation of silt checks or silt traps downstream of the work area.

The scope of work, at a minimum, will include all work required to provide an Index E simulator,
cab control devices, fuel delivery system, ramp and building modifications to deliver a project that
is fully functional for all users.
A. Existing Training System
The existing training system was installed by the Symtron Corporation in 1998. It uses
Programmable Logic Controllers (PLC) and Windows based desktop computer
controllers and Liquified Petroleum Gas (LPG) as the fuel source. The system is
specifically composed of the following:
1. The SPILL Trainer
2. The SAFT Trainer
3. 30,000-gallon LPG storage tank
4. LPG delivery system (piping, pumps, valves, thermocouples etc.)
5. PLC cabinetry, desktop PC, hardware, and software
6. Live Fire Control Tower
7. Multipurpose Building
8. Classroom and support facilities
9. Vehicle Storage Bay
All existing simulator equipment (tentatively identified as items 1-5 above) are to be
replaced as part of this project
The project involves removing and replacing brick wall areas to install thru-wall flashing above the existing roof in select locations. Additional scope involves removing all aluminum frame windows and glass block windows and their lintels and replacing them with blast resistant aluminum storefront windows and galvanized steel lintels. There is an allowance for brick tuckpointing. The brick on the building will be washed and a sealer applied. The project is located at the National Guard Armory in Hazard, KY.

The existing Breckinridge Steam Vault is a bunker style e vault that does not meet the
minimum safety requirements of Utilities. This project will remove the bunker style top and
replace it with a ground level concrete top with a hinged aluminum lid and ladder that
conforms to Utilities’ specifications. The failed exhaust system will also be replaced during
this project. The aforementioned work requires some steam and piping modifications as well.
When complete, the steam vault will allow for safe entry conditions for Utilities personnel
while also providing a more aesthetically pleasing design.

The proposed project will include up to approximately 1.4 acres of total disturbance
within project limits including approximately 5.8 acres, including two project construction sites
and one excess earth fill area. Actions to be taken under this project at the Mike Hopson Site
will consist of the reclamation of a small landslide that is directly impacting the Hopson
residence. The landslide will be completely excavated, with the earthen material being
transported to a remote waste area located off the KY-1426 ramp to/from northbound US-119. In order to facilitate removal of the landslide material that is lying against the residence, a deck structure will have to be removed and replaced. Construction debris from removal of the deck will be transported to an approved landfill for proper disposal. Temporary debris barrier is included to protect both the Hopson residence and a neighboring residence during construction. A length of 48” diameter culvert is included for replacement of an existing stream culvert beneath Thompson Branch Rd. in the event that the existing structure is damaged by construction-related traffic. At the Charles Atkinson site, efforts will be directed towards the reclamation of a subsidence hole that has developed in the front yard of the Atkinson residence.
The hole will be overexcavated to achieve a stable configuration, then backfilled with Class-II stone, topped with a 2’ thick layer of #2 aggregate, a layer of filter fabric, a 3’ thick layer of polyurethane foam, and a 2’ thick soil veneer. Any excess soil from excavation at this site will be placed within the Atkinson residential yard and covered with the existing topsoil prior to revegetation.
The project will not cause the disturbance of any forested areas or caves and cave-like
structures (specifically including underground mine openings). The only potential for direct disturbance of streams will be limited to the replacement (if necessary) of the culvert beneath Thompson Branch Road. The strict implementation, maintenance, and enforcement of standard AML Best Management Practices for Sediment and Erosion Control will prevent any significant amounts of sediments disturbed by construction-related actions from entering area streams, thereby preventing indirect impacts to aquatic resources. The construction sites will be revegetated by means of standard agricultural tillage using the standard AML Residential seed mixture appropriate for the season during which construction occurs. Seedbed preparation at the construction sites will also include the removal of any large or protruding stones that may interfere with either lawn growth or maintenance. The fill area will also be revegetated using agricultural tillage, though the season-appropriate standard AML Reclamation seed mixture will be used. Bituminous repair and roadstone are included for the resurfacing of any sections of paved or graveled road surfaces determined by KDAML personnel to be damaged by construction-related traffic at either site and at the entrance to the remote fill area.
The project consists of all the necessary work required to construct a State Veteran’s Nursing Home in Bowling Green, Warren County, Kentucky.
The site is a previously undeveloped parcel in Bowling Green’s, Kentucky Transpark, located off the I-65 Connector. The parcel is located south east of US HWY 31W along Mizpah Road. The address for the facility is 726 Mizpah Road.
The project consists of one community/administration building and two (2) residential “Neighborhood” buildings with connecting structures for a new sixty (60) bed nursing home facility. The construction type is V-A. The occupancy group is I-2 Healthcare. The community building is approximately 29,000 SF, the neighborhood building 47,000 SF for an approximate total of 76,000 gross square feet. An additional Support Building is approximately 2,600 SF. The structures are steel frame on concrete foundations with slab-on-grade floors and light gauge metal and steel bar joist roof structure.
The work includes, but is not limited to, the following: site clearing; earth moving; site utilities for electric, water, gas and sewer services; storm drainage; asphalt and concrete paving; landscaping; cast-in-place concrete; masonry; exterior metal wall panels; structural steel framing; wood blocking; finish carpentry and casework; standing seam metal roof; roofing; sheet metal flashing and trim; hollow metal doors and frames; clad wood doors; fiberglass windows; aluminum entrances and storefront; finish hardware; metal framings; gypsum wallboard; acoustical ceilings; tile; carpet; paint; kitchen equipment; fire suppression system; plumbing for gas, water, sewer; HVAC equipment, ductwork and controls; electrical power, lighting and emergency power; communication and access control systems.
This project consists of the removal of the existing play equipment, fencing, timber retaining wall, and associated features. A new playground has been designed to facilitate the developmental needs of the children at the Kentucky School for the Blind. Work includes site work to facilitate the installation of new play equipment, the reinstallation of existing salvaged play equipment, security fencing and gates, concrete sidewalks, and poured-in-place rubber safety surface.
A. Bluegrass-Oakwood is an Intermediate Care Facility supporting individuals with intellectual and developmental disabilities. Construction at this site must be performed in a manner that protects the existing staff and clients from risks that are inherent to construction operations.
B. Work of the Project includes:
1. Full tear off and legal disposal of existing asphalt shingles, plywood and insulation at Cottages 102,103,105 and 118
2. Installation of new nailable base, vented roof insulation panels, asphalt shingles and associated sheet metal flashing and trim.
3. The work also includes raising the height of existing plumbing stacks to conform to the Kentucky Plumbing Code.
4. All other work shown on the Contract Drawings and/or included in the Project Manual.
