The Kentucky Department of Veterans Affairs proposes to construct a new Veterans Cemetery on a portion of a 42.45 acre site located at 1280 KY Highway 118 in Hyden, Kentucky. The cemetery will be developed with approximately 5,400 interment – burial sites in below ground crypts, columbarium niches and in-ground cremain niches. In addition, the cemetery site will be developed with an approximate 1072 square foot Committal Shelter, an approximate 2,600 square foot Administration and Maintenance Building and an approximate 511 square foot storage shed. Access drives for circulation through the cemetery for visitors and ceremonies will be provided in addition to 12 parking spaces for staff and visitors. A maintenance yard is provided in conjunction with the facility in addition to appropriate utilities and infrastructure necessary for the operation of the cemetery facility
Work includes providing all required material, labor and equipment to perform selective floor and wall preparation and flooring and wall systems installation as hereinafter described. The project is to be performed in Men’s and Women’s Shower Rooms in Building No. 3823 (Store Side Shower House) located at the Kentucky Horse Park Camp Grounds. Work is anticipated to begin on or about August 1, 2016 and to be substantially complete within 45 days. Due to scheduling commitments of the Horse Park, bidders shall fully evaluate the scope of work and satisfy themselves that the Shower House work will be fully completed and the space ready for use within the prescribed time frame prior to submitting their bid. The scope of the project improvements includes floor and wall preparation at the shower and changing areas, including cleaning, grinding and / or etching existing tile and concrete masonry units. Associated flooring work includes stripping and preparation of tile floors in toilet and lavatory areas to receive floor & grout stains & sealers. In addition, work includes sanding and preparing existing concrete masonry walls, wood trim and drywall ceilings to receive new painting systems. Improvements include Installation of new epoxy resin flooring and wall systems at the shower areas, application of new floor stains and sealers, application of water-borne epoxy and enamel painting systems, and reinstallation of shower doors, benches, toilet and shower accessories removed to accommodate new work. Mechanical work includes modifications to the existing piping serving the showers within the shower rooms and installation of new cold water piping and wall mounted shower units at each shower location.

The project requires removal of approximately 6,200 square feet of existing monolithic rubber flooring system and 4″ high wall base located in the Gym of Woodsbend Youth Development Center and replacement with new VCT flooring and 6″ high rubber wall base The scope of the work also includes: A. Removal of approximately 132 square feet of existing concrete slab-on-grade flooring located in the northeast corner of the Gym where the slab has settled. The slab-on-grade will remain a soil bearing system on compacted fill. The slab-on-grade repair will be limited to the area at columns A.9-6, A.9-7, and D-7. The existing slab will be removed and replaced with new compacted dense grade aggregate to the existing sub-grade elevation. New cast-in-place concrete will be placed over a vapor barrier and tied to the remaining concrete floor slab with #4 dowels and reinforced with steel woven wire reinforcing mesh lapped onto the dowels. B. Preparation including skim coating if necessary of the existing floor slab to receive new vinyl composition flooring. C. Installation of new 1/8″ vinyl composition tile (VCT) flooring including laser cut tile play lines for basketball and volleyball and application of seal and wax coatings. D. Accessories necessary to ensure level/ADA compliant transitions to all areas where the VCT flooring will butt existing concrete, ceramic floor tile and/or VCT floor finishes. Prevailing Wage Law (KRS 337.505 – 337.550) is NOT applicable to this Project.

The project also includes 26 push / pull pits; reconnections to distribution system with 6”, 12”, 20” and 24” ductile iron pipe; supply and installation of valves; restoration of pavement, curbs, drives, sidewalks, lawns, landscaping, trees and placing sod for the golf course.
