The project scope:
This project is for the cleaning, sanitizing and encapsulation of commercial heating, ventilating and air conditioning systems located within the WKU South Campus.

The proposed project (approximately 2.0 acres total) consists of drainage controls in Perry County. The project area is located on the United States Geological Survey (USGS) 7.5’ Hazard North quadrangle (see the attached project and site maps).
The James Cheek Site consists of installing a subdrain, a concrete block-tied mat, and pipes around the Angie Back residence. The site is located at 28 Tyler Lane, near the community of Bulan, KY. The project area is centrally located at N 37° 19’ 19.20”, W 83° 9’ 6.80”. To reach the site from the junction of Hwy 80 and Hwy 1146 east of Hazard, turn right onto Hwy 1146. Travel approximately 1.2 miles to Hwy 476. Turn left onto Hwy 476. Travel approximately 1.6 miles to Tyler Lane. Turn left onto Tyler Lane. The first house on the left is the Angie Back residence.
Reclamation work consists of installing 285 L.F. of subdrain and finger drains behind the residence, installing 160 L.F. of concrete block-tied mat along the bottom of the hillside, and replacing an existing pipe across the yard with a new 18” HDPE pipe outletting into the existing drain. Class II rock will be placed at the pipe outlet to reinforce the side slopes. Also, a new 18” pipe culvert will be replaced across Tyler Lane road. The existing driveway will serve as the access road and it will be replaced with roadway stone after the construction work is completed. The waste material generated during project work will be used on site as indicated on the site map within the project boundaries.
During the process of these construction activities, trees over 5 inches in diameter at breast height will not have to be removed in the AML problem area and to install drainage controls. A sediment and erosion control program, consisting of such measures as constructing hay bales/silt fence to be maintained throughout the life of the project and prompt re-vegetation using agricultural limestone, fertilizer, seed, and mulch will be implemented on all areas disturbed by this project.
Entry to the work areas will be accessed by the county road, residential driveways, and residentially maintained yards. The entire project work area slated for any excavation has been previously disturbed by any or all of the following: coal mining operations, including mine drainage, timber operations, gas/oil well development, residential development, road construction, and/or high velocity water flows associated with heavy rain events and flooding. These disturbances consist of significant upheaval, mixing, and removal of earthen material from deep excavation, grading, sub-surface drilling, fill material placement, and erosion. There should be no undisturbed earthen material to a depth of several feet at the areas slated for significant project-related construction activities.
This project will include pre- and/or post- project maintenance and repair of existing previously constructed public county roads utilized during the construction of this project, which are administered by the county or local road authority. This construction activity will be conducted under the authority and supervision of the local public road authority and will consist of standard road construction practices appropriate to mitigate impacts to the local community from potential degraded road conditions in order to maintain public traffic safety.
Description and location of Project: This unit based construction order is for a 12-inch trench drain, pipe and drain, and driveway replacement along Shadwell Lane in Metro Council District 16.

•Replace the joint filler and silicone water seal between each flood wall monolith
•Remove graffiti and repaint
•Seal every monolith and gate well with anti-graffiti sealent
•Remove vegetation along flood wall and any overhanging trees

Description and location of Project: This unit based construction order is for the Infrastructure Completion Project in Beech Spring Farms in Metro Council District 16.
The project involves the complete removal of the existing DX HVAC system. This will include removal of the existing hot water boiler system, expansion tanks, housekeep pads, circ. Pumps, air separator and controls.
This project will include the installation of a new VRF systems, mechanical ventilation system which includes an energy recovery unit, ventilation air ductwork and the installation of the refg. piping. This contractor will need to make all of the interlocks between equipment.
If Add Alternate #1 is accepted will paint the Drill Walls, windows, doors and frames.
If Add Alternate #2 is accepted install new Epoxy floor finish in the Drill Hall.
Also, this project has a $39,000.00 allowance to expand the existing DDC controls to except the new HVAC equipment.

The Lewis County New Central Elementary School project will be bid and constructed utilizing the
Construction Management Agency form of construction contract delivery. Trace Creek Construction will
be the Construction Manager for the project under direct contract with the Lewis County Board of Education. The New Elementary School project is generally described as the construction of a new 2-story facility, including site improvements and demolition of existing buildings.

PROJECT DESCRIPTION:
Camp Ground Elementary School located in London, Kentucky for the Laurel County Board of Education is being renovated to provide new finishes throughout the building interior including new floor finishes, new ceilings, new lighting, new fire sprinkler
system and upgraded HVAC system throughout to become a 300-student elementary school.
Renovations include demolition of existing toilet rooms to become classrooms and the existing Media Center to become a new Music Room, new Art Room, and Resource Rooms to meet Kentucky Department of Education standard guidelines for a 300-student capacity facility.
There will be two new additions and an infill of an existing exterior breezeway to become a new
enclosed corridor. The addition at the front entrance to the building will include a new secure vestibule and reception area, with a new Family Resource Center and new Media Center. The second addition at the rear of the building will include a new vestibule and entry lobby into the existing Gymnasium complete with new toilet rooms for students and staff. The new lobby will provide second direct access to the existing gym from the interior of the building that was
previously accessed via a single interior entry point within the existing building. The project will include improvement, upgrades and reconfiguration for the vehicular access for both student drop off by parents, a new bus loop and staff parking. There are several new exterior covered areas to provide additional staging locations for student pick-up and drop-off There will be an
alternate amongst others included in the project for a complete re-roof of the existing building as well.

The Work consists of replacing two (2) existing 10’x10’ sliding motorized gates, operators, 2 personnel swing gates, and modification of existing fencing. Two existing gate controllers shall be reused and modified as required for the new gate operators. Replacement gates will be motorized sliding 14’x14’ each, swing gates will be 7’x3’ and new fencing to match existing. Replacement of a significant area of concrete in the gate area is included along with all concrete and metal supports, bollards as indicated on the drawings.
Electrical circuit and replacement of electrical circuits during construction is required.
Perimeter fence detection system removal and reinstallation will be performed by LLCC.
The project shall be constructed in two phases so that one secure gate remains in operation at all times.
