PROJECT DESCRIPTION: The project scope includes roof replacement work at both Estill County Middle
School and Estill Springs Elementary School. The scope of work at Estill County Middle School includes the replacement of approximately 63,000 square feet of shingle roof with a metal roof panel system. The scope of work at Estill Springs Elementary includes the replacement of approximately 38,500 square feet of shingle roof with a metal roof panel system, the replacement of approximately 10,500 square feet of EPDM roofing with PVC/polyiscyanurate roofing system, and also the
installation of a roof panel system for a new steel gabled canopy which will be constructed by others.
The Contractor will also need to coordinate work and schedule as it relates to the exterior canopies
with the Estill Springs Elementary Renovation project; hired under a separate agreement by the School District.

The project scope includes the replacement or an existing HVAC system throughout the building, a renovation to add administrative space. enhancing the secure entry vestibule, and restroom renovations. Exterior improvements include a new parent drop off road
extension, new canopies at the main entrance and parent drop off entrance, and site drainage
improvements. The Contractors will also need to coordinate work and schedule as it relates to the
exterior canopies with a separate roofing contractor, hired under a separate agreement by the School District.

PROJECT DESCRIPTION:
The Medicine Clinic project will include the interior renovation of approximately 26,275 square feet of a
working outpatient clinic. Work includes, but is not limited to, demolition (gutting), gypsum board and
metal stud walls, gypsum board ceilings and soffits, acoustical ceilings, wood doors and hardware,
hollow metal door frames, glazing, specialty glazing, automatic operators, resilient flooring and base,
carpet tile, tiling, painting, custom millwork and solid surface counters, chair rail and corner guards, wall
protection, specialties, ductwork, lighting, electrical, data, fire alarm, cable tray, plumbing fixtures and
piping, and fire suppression. This work will allow for new exam rooms, enhanced patient and staff
circulation, and improved waiting areas. Upgrades to the existing HVAC will bring it to current UK
standards.

The Dan Walker residence is located in southeast Harlan County, near the Cranks Creek Wildlife Management area at 36° 46’ 19′ N and 83° 10’ 13′ W. The site can be reached from Harlan by traveling east on US 421 for 10.7 miles to the Cranks community. Turn left onto KY 568 and travel 0.4 miles to Sulphur Springs Road on the left. The Walker residence is 0.5 miles on the right (239 Sulphur Springs Road).
Site preparation includes clearing and grubbing of vegetation. Straw bales and silt fence are included to minimize erosion from the site. No trees larger than three inches in diameter, whether dead or alive, will be removed except during the designated time allowed unless they are on an unstable slope.
A slide has occurred beyond the western end of an existing reinforced concrete wall. Slide debris and dangerous perched rocks were removed in Phase I. Additional reclamation plans consist of constructing an additional length of reinforced concrete wall with key and weep holes. This wall will be tied in to the existing wall and will be “winged” as necessary to aid in holding back the Class II fill material. A length of Debris Barrier Wall is planned to protect the residence from sliding or rolling debris. Drainage will be improved with retaining wall subdrain and subdrain, and a ditch on the slope above the slide will be repaired with ECB and Class III as determined by the Engineer. Chain-link fence
Utility relocation is also included as may be needed during construction of this project, and roadway stone is included to provide access as necessary. Geogrid is included to be placed beneath the wall and for access as necessary. Filter fabric is included for wall construction. ECB and Class III Stone are included for ditch repair.
Waste is intended to be placed in one or both of two designated waste areas. The Primary Waste Area is located northeast of the site at 36° 46’ 55’N and 83° 09’ 35’W and can be reached by following KY 568 north to a left turn onto Skidmore Road. A Secondary Waste Area is located southwest of the site at 36° 45’ 30’N and 83° 11’ 55’W. It is anticipated that the total area from both waste areas that will be used is less than one acre, and site preparation at the waste areas should be minimal.
All areas disturbed by construction activities is to be revegetated in accordance with regional, regulatory, and project standards, using agricultural limestone, fertilizer, seed and mulch. A standard seed mix is to be used.
PROJECT DESCRIPTION:
The project consists of cleaning and scaling cracks on Runways, Taxiways and Roadways to prevent water from entering the pavement.
The project is to provide an exterior hydraulic dock leveler installation at the newly constructed C-1 Garage located at, 1239 Wilkinson Blvd., Kentucky Transportation Cabinet, Frankfort, Kentucky. The hydraulic dock leveler is pit mounted into the existing loading dock located in the Northeast corner of the C-1 Garage, so partial demolition and removal of existing reinforced concrete apron and dock face is necessary (approx. 143 SF) to prepare a space for the pit. Work generally consists, but is not necessarily limited to, the following: Provide work area safety and security at the loading dock. Removal of all building site improvements as described in the documents. Disposal of all demolished building materials in an acceptable manner. Prevent gravel from washing out beneath adjacent slabs. New 8” thk. reinforced concrete pit walls. New 8” thk. reinforced concrete pit slab. Provide embedded pit steel per documents. All new concrete work shall be doweled into existing adjacent concrete per details. Provide indicated hot dipped galvanized pit steel per document size and manufacturer requirements. Installation of a new hydraulic dock leveler per specifications. Confirm and coordinate hydraulic dock leveler, pit steel and pit size to meet manufacturers requirements. New electrical supply and disconnect per drawings and specifications. Provide user group training, warranties and O&M Manuals. Provide new loading dock barrier chain & bollard safety system.

The project scope consists of a new office facility for Comprehend Behavioral Health. The new facility is located directly adjacent to an existing medical office building at 611 Forest
Avenue. The new facility will consist of Office Assembly as well as primary care exam
rooms to support the owners program. The new facility will be 3 levels at an approximate
25,303 sq/ft cumulative area. Ground level is to be constructed of concrete frame with a mat foundation and slab-on-grade above. Levels one and two and the roof are to be constructed of
Steel frame, both levels will be above grade. Exposed exterior finishes to include insulated composite aluminum panels, brick veneer and aluminum curtain wall. Site features will
include drives, parking, landscaping, concrete and paver areas and retaining walls. Mechanical,
plumbing, fire protection, electrical and data systems are included in the project scope. Please

PROJECT DESCRIPTION:
This project will include the removal of the 16,875.00GSF temporary dining facility know as Bowman’s Den from the campus of the University of Kentucky and return the site to its original condition. Constructed in late 2014 and becoming operational in 2015. Bowman’s Den services students with retail food operations while the Student Center was undergoing significant renovation and rebuilding. The entire structure foundation and limited
infrastructure will be removed and the area returned to conditions prior to the construction of
Bowman’s Den. The area will be sodded and an irrigation systems, similar to the one previous existing installed. No Planting are required.
