The project includes, but is not limited to grading, building erection, asphalt paving,
pavement marking, seeding, sodding, and temporary erosion control measures as
shown on the plans and indicated in the specifications.
The project consists of milling, pavement patching, pavement
placement, curb ramp replacement, curb, sidewalk and driveway replacement, pavement striping, and
other appurtenant work associated with pavement improvements to various streets throughout the City.
The boiler house was built circa 1937 and renovated in subsequent projects. The boiler house is slated for eventual demolition by others, but the scope of this project is to abate regulated asbestos containing materials from the interior and exterior of the boiler house.
The regulated ACMs include friable thermal insulating materials in the boiler walls and on piping and associated fittings and joints. An asbestos survey has been completed for this project.
Abate Asbestos From 3 Boilers Remove all asbestos from the fire box (walls, floors, and tops), steam drum, mud drum, and any other component considered part of the boiler system. Abate Asbestos From Piping and Associated Fittings Remove all asbestos from all piping, elbows, valves, breeching, ductwork and any other fittings.
Manage, transport, and dispose asbestos containing materials as a friable waste in a permitted landfill. Contractor to comply with all authorities having jurisdiction including but not limited to those of the Kentucky Energy and Environment Cabinet.
The project includes the construction of an approximately 1.0-acre asphalt parking area. The scope
of work includes light excavation, storm drainage, asphalt paving, concrete sidewalk and curb, and
other associated appurtenances.
– Asphalt overlays will be conducted on 1st and 2nd Street aprons. The total area for 1.5” asphalt overlay is approximately 194 SY.
– Asphalt mill and inlays will be conducted on Martin Street and W Stephens St.
The total area for 1.5” mill and inlay is approximately 5442 SY.
– Asphalt paving will be conducted in the Midway Cemetery. The total area for 8” DGA Compaction, 2” base course, and 1.5” surface course paving is
approximately 228 SY.
Project scope includes removal and reconstruction of two exterior stairs, asphalt pavement overlay and minor repair, and an Additive Alternate to providing a permanent, ADA compliant ramp for access to the front door. The existing asphalt pavement with be cleaned and prepared for a new surface overlay.
At the front entrance, Additive Alternate No. 1 proposes the existing demountable ramp will be removed, and non-compliant section of sidewalk also removed to replace these with a permanent, ADA compliant access ramp from the parking area to the front door. Minor concrete patching at the other front door is also indicated as part of Additive Alternate No. 1.

a. The Jimmy Hall Bridge HP AML Reclamation Project consists of removal and replacement of the bridge to the Hall residence of Jimmy Hall near the community of Van, KY. The project site is located at Latitude: 37°07’56” N, Longitude: 82° 53′ 52″W. To reach the site from the KY AML Branch Office at 101 Bulldog Lane, Hazard, KY turn right onto KY 15 south and travel 25.4 mile to the intersection of KY 160, turn right onto KY 160, travel 2.1 mile to house on left of road and below the road elevation across the stream.

A low water crossing from the Barbara Ison Slide HP PH2 AML Reclamation Project washed out and debris became lodged beneath the bridge and contributed to the damage of the structure. It is proposed to remove the bridge and abutments and construct a permanent crossing with a portable timber bridge. Sediment control will be provided by installation of silt fence or bales as directed. The material from the existing abutments will be excavated to provide stable approach and any excess will be place at the previously used waste area for the Barbara Ison project.
No trees greater than 3″diameter will be removed during this phase of reclamation.

Waterline Replacement Project which will consist of installing approximately 1,700 LF of 8” Class 250 PVC waterline in
Estill Bottom in Floyd County, Kentucky.
