Health Hospital. The project will consist of the construction of an indoor/outdoor Four Seasons Room as

planters with wood capped bench seating as well as an aluminum storefront window wall with two
nanawall door systems. The Four Seasons Room will also consist of a glazed sloped roofing system and
glue-lam beam structural framing. The exterior courtyard will consist of two free stand stone veneer
planters with wood capped bench seating and structural steel columns that will support glue-lam trellis
framing above. The exterior courtyard will consist of additional stone veneer planters with wood cap
bench seating throughout the courtyard as well as a glue-lam trellis that runs along the east wall of the
existing courtyard. Finishes include stone veneer for planter walls, p.t. wood for planter wall caps and
benches, black powder coated structural steel, and p.t. stained syp. glue-lam beams.

Electrical Work Scope includes but is not limited to the following New ductbanks for primary power and communications site distribution with connections to existing manholes. Installation of new University-owned 15 KV feeders, new primary metering risers and terminations to existing cable infrastructure. Coordination with communications service providers for installation of fiber optic and copper cables. Installation of a new ductbank and riser for Kentucky Utilities-owned primary cables. Relocation of existing site lighting and provision of new building-mounted site lighting.
Site Work Scope includes but is not limited to the following: Site clearing and grading associated with utility work and installation of a new gravel utility pad with new 8′ galvanized chain link security fencing and gates, restoration of disturbed turf areas via hydroseeding.

The proposed Middle Beech Creek AMD abatement project AKA Pond Creek Bond Forfieture consists of 115 acres and includes acidic and high iron discharge. This site includes a title V bond forfeiture permit. The site is located near Beech Creek in Muhlenberg County, KY at Latitude 37° 10’21” N and longitude 87° 04’11” W. To reach the site from the Central City exit on the Western Kentucky Parkway, take US 431 South through Drakesboro for 6.2 miles, then turn right onto KY 172, next, go 2.2 miles, then turn left onto KY 246-20, then go 150’, turn left onto Merle Travis Highway, then go 2 miles, then turn right onto Beech Creek Road, go 0.3 miles, then turn right onto Wyatt Cemetery Rd.
Proposed are three class III diversion ditches with limestone sand, 4400 linear feet total. Also proposed is covering and reseeding bare areas within the 115 acres of the site. The rock in the ditches will be high purity limestone. Elimination of pit(s) will be part of the project scope, to treat then release water once up to defined standards as called for in AML technical specifications.
Silt control will be established prior to construction activities except for site prep for access. Silt control will be a mixture of silt traps and/ or straw bales and silt fences. Additionally, best management practices will be utilized to the extend practical.
Vegetation on the site consist of a mixture of grassland and trees. Removal of trees will be minimized to the extent practical. However, Trees will be cut when necessary to complete the proposed construction activities. All disturbed areas will be promptly revegetated at the end of construction.
