This project is for the construction of a 80’-3” long x 62-0” wide x 10’ high concrete structure with an interior concrete pad and concrete entrance apron for the purpose of bulk storage of roadway, deicing salt.
Class “A” concrete to be reinforced with epoxy coated rebar & fiber, which shall be used throughout the shallow, spread footing and walls. The Contractor shall be required to perform all excavation, filling, grading, & compaction of sub-grade and sub-base necessary for the construction of footing, interior pad, and entrance apron.
Concrete walls will be ten (10) feet high, will be twelve (12) inches thick, will be constructed on the concrete footing, and will support a pre-engineered, membrane covered (roof) structure (PMCS) with trusses being fourteen (14) foot on-centers. The roof structure is to be furnished and installed separately by Bulk Storage, Inc., per their current Master Agreement upon completion of the footing and walls under this contract. The (PMCS) is not a part of this Scope of Work.
Nine (9) inch thick, Class “AA” concrete modified with fiber & epoxy coated rebar, concrete slabs shall be placed inside the footing and at the entrance on the exterior of the structure.

This project encompasses repairs to several exterior concrete elements at Hazelwood Intermediate Care Facility. The South Stair at the Employee Parking Lot will be removed and replaced in entirety, along with a new surface-mounted handrail. At the Dumpster area, the concrete dumpster pad, along with the perimeter asphalt, will be removed and replaced. Two engineered rubber curbs will be installed in this location. The West Ramp will have its topper slab removed and replaced, and receive a new surface-mounted guardrail and handrail. The North Ramp will have its topper slab removed and replaced, and will have the existing stair removed and replaced. Both the ramp and the stair will receive new surface-mounted guardrails and handrails. At the Loading Dock, four structural steel HSS columns will be removed and replaced, and will received new footings. The existing guardrail will be removed and replaced with a surface-mounted guardrail.

Administration (OSHA), and other regulatory agencies to complete removal of Mercury-containing
materials.

Task 1. Remove and dispose of approximately 7,000 square feet of Mercury-containing flooring
throughout the Auxiliary Gym (Middle School Gym). Work includes removal of all materials on top of the concrete slab.

Note: Metal volleyball stanchion post holders and clip holders to remain in floor.

Task 2. On top of the remaining concrete slab, attach the following label every 10 feet on the floor: “
Danger Potential Mercury Hazard in Concrete. Do Not Disturb”. Upon completion of flooring removal and successful clearance testing. Install two air-tight layers of 10 mil fire-resistant polyethylene sheeting over the entire floor. Seal sheeting with duct tape and spray adhesive 1” up and along each wall and between each section of sheeting.

Task 3. While Auxiliary gym is in the regulated work area, clean HVAC and ductwork that services the
Auxiliary Gym, Room 168M, and Room 168 M Storage.

ARCHITECTURAL
The Hazard Community and Technical College (HCTC) Rad-Tech renovation project primarily consists of the approximately 400 square feet selective demolition and renovation of existing shell space on the first floor of the J. Marvin Jolly Classroom Center in Hazard, Kentucky into a Radiology Training Lab and Control/Observation Room. The build-out of the new Radiology Training Lab includes the installation of a new X-Ray, lining of the room with lead-lined gypsum board, a new lead-lined door, lead protective viewing window, resilient flooring, acoustical tile ceiling and storage cabinets. The Control / Observation Room receives a new solid core wood veneer door to match existing along the corridor, built-in workstation, resilient flooring, and acoustical tile ceiling.
This project includes providing HVAC and electrical systems to accommodate the new Rad-Tech Lab area in the Jolly Classroom Building.
STRUCTURAL
The structural scope of the project consists of supporting the new X-ray equipment. The equipment is being supported by a system of unistruts which are then supported by (2) structural steel beams. These beams are being hung from the existing structural steel beams with hollow structural steel posts and braced with unistruts to prevent sidesway. The X-ray equipment has a very stringent deflection criteria which is unrealistic to meet without providing completely new structural support systems. It was decided that the existing support system would be sufficient to support the hanging mechanism. The primary support system as shown will provide the deflection criteria as required; however, the existing building which ultimately supports the X-ray equipment does not.
HVAC
A new VAV box with hot water re-heat will serve the X-Ray room. It will tie into the existing supply air duct and the existing boiler hot water loop. The Control Room will be served by the existing VAV box. Ductwork will be added/modified to accommodate this area.
ELECTRICAL
New lighting and power to room fit up. Electrical connections to X-ray equipment.
