The work to be bid upon includes furnishing and constructing two vertical pumps, control panel, building, electrical services, water line piping and all other necessary items
The entire South and East Façade is to undergo a complete masonry restoration including necessary masonry repairs, tuckpointing, joint sealants to continue the cleaning and restoration work that is underway on the North and West façades. All windows on the South and East façades are to be re-caulked to existing masonry openings. After the restoration, the façades are to be cleaned of all biologic growth, carbon crust, dirt, grime, etc. New bird control devices are to be installed.

PROJECT DESCRIPTION:
The modernization consulting services work consists of administering the planning/design, implementation and construction management of the Agency’s Comprehensive Grant/Capital Funding and DECD or other modernization\development programs to assure that the program is implemented in full compliance with all applicable Federal and State regulations. The work of the modernization consultant shall include, but not be limited to:
Annual planning process and HUD submissions;
Bid request, review, and recommendation of architects/engineers construction drawings and specifications;
Obtaining HUD approval at each phase of the work as required;
Scheduling the Comprehensive Grant/Capital Funding Program and submitting a copy to be approved;
Put bids on the street for projects.
Field inspection to determine physical needs;
Scheduling advertising and conducting of tenant/public hearings for community input;
In consultation with the HADR Physical Plant Manager, review Work Orders for repetitive repairs which should be deferred to modernization and
Preparing a Monthly/Periodic Status Report for the Executive Director and a Quarterly/Periodic Status Report for the Board of Commissioners regarding the progress of the Comprehensive Grant/Capital Fund Programs.
Preparing required documents and consulting with the HADR’s Executive Director to perform Line of Credit Control (LOCC) transfers, and to submit to HUD
Monitoring & evaluating modernization pertaining to Energy Performance Contracting (EPC)
The modernization consultant will devise a management plan that consists of an overall management strategy to provide administrative control for the entire project from program development to project close- out and final completion. The plan is developed in collaboration with the Executive Director. The plan includes: project definition, objectives, work statement, organizational chart, schedules and cost controls
The Work consists of site and building demolition, wall and floor construction for the installation of a three-story hospital grade elevator. Work also includes limited upgrade of electrical service. All work shall fully comply with applicable codes and regulations for psychiatric hospitals

The Project consists of constructing 8,050 linear feet (LF) of 16-inch ductile
iron water line, 1,280 LF of 6-inch PVC water line, and all associated valves, services, and
appurtenances.

The project is located on KY30, 2.6 miles west of Jackson at the Department of Juvenile Justice Breathitt County facility. The project is the result of a slide located between the two buildings and an open portal. The slide has taken out a portion of an access road. Also, some of the cracks and breaks from the unstable area are located above a greenhouse.
The project area is 3.48 acres and the construction area is 2.50 acres. The project consists of construction of a non-reinforced concrete block wall- type II, drainage controls, and a wildlife accessible portal closure. Silt control measures at the site shall be a silt fence, installed and remaining in effect until construction is completed at the site. No trees will be cut as part of this project.
Drainage controls for these sites shall consist variously of Class II/III ditches, ECB ditch, Class II/No. 2 Stone backfill, Subdrains etc. Bituminous repair, including roadway stone, has been factored into the contract. Revegetation of the site, including agricultural limestone, fertilizer, mulch and seed will be done according to standard AML design specifications.
PROJECT DESCRIPTION:
North Fork Valley Community Health Center’s (NFV) Health Infrastructure Investment Program (HIIP) project proposes to provide many needed repairs and upgrades to the now forty-two-year-old facility that is home to the June Buchanan Medical Clinic (JBMC) site. Reconfiguration of the patient registration and pharmacy areas will provide much needed improvements to patient flow and compliance with the Americans with Disabilities Act (ADA). Conversion of space will create two new exam rooms and upgrades to the flooring will be installed in the entire facility to replace the old and worn vinyl flooring currently installed. Restrooms will be renovated to provide better accessibility to patients and compliance with the ADA as well. Also in the interest of patient accessibility, the entry way doors will be removed and replaced with automatic open and close doors. Safety and security upgrades are also planned with the installation of a new security system to provide monitoring of activities both inside and outside of the clinic and the installation of a new sprinkler system. Upgrades to the exterior include EIFS trim to the building façade. Lastly, the roof which has experienced leaking issues will be replaced with a new Thermoplastic polyolefin roof.
The scope of this project is to repair the cast in place concrete and the precast concrete elements that are supporting the guard rails. The guard rails are required to be cutoff and reinstalled. Over drill the existing core holes to remove embedded portion of the guard rails. Remove any loose, partially spalled or delaminated pieces of the cast in place concrete or the precast panels and patch these areas. Epoxy pressure inject any remaining cracks in the concrete or precast elements. The guard rails are then to be reset. Replace the failed expansion joint.
There is one alternate in the project which is clean out and reseal the joints between the precast and the cast in place concrete.
Work of the Project includes replacement of certain HVAC equipment serving areas of the CPTU Building at the Kentucky State Reformatory located at 3001 West Hwy 146, LaGrange, Oldham County, Kentucky. Demolition of existing HVAC systems (by Owner). Installation of packaged air cooled chiller, cooling coils and piping. Modifications to existing air handling systems. Installation of temperature control systems. Electrical wiring of HVAC equipment.

The project includes the construction of a 1,500+/- square foot patio picnic shelter next to the existing Paddock
Café concessions building. The proposed structure is an open air type building with stone veneer columns on a
concrete slab. The roof is constructed of pre- engineered wood trusses on LVL beams spanning the columns.
The work includes limited site work, excavation, concrete footings, masonry foundation walls, stone veneer
columns and wood framed roof system. The electrical lighting and power systems are rough-in only. The exterior
finishes include cement panel board and batten ceiling and asphalt shingles with cement panel soffits and trim.

This project will address drainage problems caused by drainage from the coal seam behind the property located at N37.58426º, W82.25032°. The site is adjacent to the Jessica Hackney Drainage AMLRP.
Install an 18” RCP culvert under the county road and concrete headwall into the stream bank. Bed the RCP in flowable fill and install the concrete headwall recessed into the stream bank and below the existing grade. On the upstream end, install a concrete dropbox. There is a gas line running along the county road. Place the concrete dropbox between the gas line and the county road if possible. From the concrete dropbox back to the hillside, install a perforated subdrain ending in a plastic dropbox. Geo-grid and filter fabric are included to construct a temporary access to the backyard. Across the backyard, construct a combination ECB ditch and subdrain tied together at the plastic dropbox. Excavate the wet muck along the hillside behind the subdrain. Excavate and attempt to open a reported closed and covered mine entry. Install subdrain into the entry area (regardless if open or collapsed) and backfill with class II. Tie the portal drainage into the backyard subdrain via a plastic dropbox. Construct a 2:1 gabion wall and backfill with class II. Set the gabions on a class II backfill and bi-axial geo-grid pad.
The project waste area is the former Johnny Coleman project waste area along US 119. The earthwork bid item includes equipment necessary to transport and spread the material at the waste area. Use haybales prior to onsite disturbance to establish silt control. Landscape allowance, utility relocation/repair, and structure removal/replacement allowances are included for disturbance to items pre-approved by the ENGINEER.
