PROJECT DESCRIPTION:
Project shall be bid to single prime General Contractor and consists of renovation and refresh of approximately 10,400 SF of existing interior ambulatory care space located on the first floor of the Kentucky Clinic. The project is considered a major renovation, with approximately 80 percent of interior partitions removed for new layout and nearly all interior finishes, fixtures and hardware being updated. New plumbing, HVAC, systems piping and electric branch systems shall be installed, utilizing existing mains. Construction shall be sequenced to accommodate pre-phasing areas prior to full demolition to maximize Owner access to existing operational Physical Therapy and X-Ray functions.
The project includes two (2) Alternates: ALTERNATE #1: FAN ARRAY
If accepted by the Owner, contractor shall schedule this work in a normal and acceptable sequence to ensure that the AHUs are fully operational with little or no downtime to the Owner. All sequencing and duration of work will be clearly communicated to and approved by the Owner prior to beginning any of the work in this alternate.
ALTERNATE #2 – Redundant VFDs
If accepted by the Owner, contractor shall schedule this work in a normal and acceptable sequence to ensure that the AHUs are fully operational with little or no downtime to the Owner. All sequencing and duration of work will be clearly communicated to and approved by the Owner prior to beginning any of the work in this alternate.
The work consists of: Upgrades of the existing security control system to a mouse driven Graphical User Interface (GUI) computer workstation and Programmable Logic Controller (PLC) based security control system. The system will include interface with existing lighting control relays and analog CCTV system. The system will include new control console (millwork), new locking control system, new digital intercom/paging system and interface with the same and new lighting control interposing relay interface. Three control workstations will be provided, two in the Restrictive Housing Unit control room and one in Tower 11A. The work will result in a fully operational security control system including all demolition, materials, installation, testing, training and warranty as provided in the contract documents.

The Neil Ratliff AML Reclamation Project, approximately 1 acre total, is located in the Broad Bottom Quad, Pike County at coordinates N 37° 31’ 12”, W 82° 33’ 23.4”. Driving directions to the site are as follows: from the route 80/23 intersection in Prestonsburg, take Route 23 South for approximately 17.6 miles. Ratliff Branch is on the other side of a concrete road barrier and is only accessible from the north bound lane. Go to the red light near Kentucky Power, and make a U-turn. Drive back less than 0.1 mile to Ratliff Branch Road on the right. Go 0.3 mile to 364 Ratliff Branch Road. The residence is on the left.
There is an eligible mine pond approximately 150 feet above the residence. Drainage from the pond is more than the spillway pipe can handle causing runoff to erode the hillside and rut out the landowner’s driveway. The road side ditch along the driveway has silted up and water ponds in their yard. AML plans to remove the dam, draining the pond slowly, and then grade the dam material into the pond allowing for positive drainage. The drainage pipes in the yard and crossing the road will be replaced. All waste material will be used onsite to remove the pond.
Construction disturbances will be kept to a minimum through the use of a stringently formulated sediment and erosion control program. Silt Barrier – Bales & Fence, rock checks and a gabion weir will be installed and maintained at all disturbed areas. Agricultural Limestone is included to ensure water quality of the pond. All disturbed areas will be promptly re-vegetated at the end of construction. Traffic Control is included to advise motorist of the reclamation work. Trees over 3” in diameter at breast height will require removal to access the pond.
Access to the site is via the county road and an existing access route void of vegetation. This project will include pre- and/or post- project maintenance and repair of existing previously constructed public county roads utilized during the construction of this project, which are administered by the county or local road authority. This construction activity will be conducted under the authority and supervision of the local public road authority and will consist of standard road construction practices appropriate to mitigate impacts to the local community from potential degraded road conditions in order to maintain public traffic safety.
Project consists of installation of masonry welding booths, exhaust system ductwork and extraction arms, filtration equipment, and distributed compressed gas piping systems for new welding program. A storage area will be upgraded to include four new welding gas regulator stations with gas cylinder storage area. Project also includes installation of UV protective window film shops interior and exterior windows. Most work will be in an existing shop that will require minor demolition of compressed gas and electrical to make way for the new work.

The proposed Mona Baker II AML project (approximately 13.4 acres within the project limit, of which only a portion is expected to be disturbed) consists of regarding a slide area which has broken out below a drive that is upslope from the Baker residence. An existing pipe across the gravel drive will be removed and drainage improved by constructing a hoe ram ditch which will take water into a new pipe, with headwalls, put in place to replace a rusted existing pipe. Existing subdrain behind the upslope residence will be extended into the hoe-ram ditch. Gabion flat bottom ditch will be placed below the pipe outlet, and a gabion weir will be constructed below the outlet before water enters the existing concrete drain. Work below this level will consist of cleaning out existing drainage structures. Site preparation is intended to include the clean out of some existing ditches and traps.
Additional work upslope will include closing one (1) portal and constructing a concrete block tied-mat ditch to take drainage into the existing drain (to be cleaned out).
An additional six (6) portals will also be closed in an area further upslope. Combination Rock Ditch-Drain will be constructed, and a drop box will be constructed where necessary to maintain positive drainage. Pipe will take water from the drop box into an excavated rock ditch/concrete block tied-mat ditch into existing drainage. A temporary silt trap type B is anticipated in this drain.
To reach the project from I-75 at the London/Manchester exit, turn left onto KY 80/Russell Dyche Memorial Highway, which will become the Hal Rogers Parkway and continue for 44 miles. Take a slight right toward Hyden, and proceed for approximately 3.5 miles to a left onto KY 421/KY 80. Proceed for approximately 0.5 mile to Main Street/KY 421 and travel a short distance to Oak Street on the right, just beyond Frontier Nursing Services. Travel up Oak Street to the site at 37° 09’ 38” N; 83° 22’ 23” W.
Additional work to be included under the Mona Baker II project will be at the Frontier Nursing School, located nearby at 195 School Street. To reach the college from The Baker site (Oak Street), turn left onto Hwy. 421 and proceed a few tenths of a mile to a left onto Hickory Lane and continue to a switchback on the right onto School Street. The site is located approximately 37° 09’ 57.6” N; 83° 22’ 45.1” W. There is a total of 1.6 acres within the project limit, but it is assumed that less than one acre will in fact be disturbed. Work at the Frontier Nursing School will consist of regarding approximately one acre of slide, and adding a variety of ditches as well as subdrain to reduce saturation problems which have added to slope instability. In addition, seven (7) test borings will be drilled as indicated in the planview.
To reach the waste area, from Hyden, take Hwy. 421 (Main Street) south for about ½ mile and turn left onto Hwy. 80. Continue about 1 ¼ mile to the waste area on the right at N 37° 08’ 56.2”, W 83° 21’ 13.4”. Material is to be placed as directed by the Engineer, and none is to be placed closer than 10 feet to the top of the bank. Silt barrier is to be installed prior to placing any fill.
Materials resulting from any tree cutting are not to be placed in the waste area, but are to be chipped on site or otherwise disposed of per the property owner’s wishes. Additional roadway stone is included to maintain access to the waste area.
Utility relocation and replacement, landscape allowance, dense grade aggregate and roadway stone are included as may be needed.
A sediment and erosion control program, consisting of such measures as hay bales, silt fences, and prompt re-vegetation will be implemented on all areas disturbed by this project. All items related to erosion control must be properly installed and well maintained throughout the length of the project.
The overhead electric and communication distribution systems serving the Western State Hospital
will be removed and replaced with underground systems consisting of manholes, duct-lines, and padmounted
transformers. Building electric services will be replaced with underground services. An
Electrical Services Building will be constructed at the Central Utility Plant and include
switchboards, panelboards, and distribution equipment; Patient Care Buildings will be served from
the Electrical Services Building. A generator will be installed at Building 20 to furnish backup upon
loss of normal power. Electric consumption will be monitored by a Central Monitoring System
included in the Electrical Services Building.
The Project will include the following add alternate bids –
a. Add Alternate #1. Repave the main drive between Building 20 and Building 63.
b. Add Alternate #2. Extend underground electric and communications services from the Campus
systems to Building 78/79.

The project involves the replacement of the existing asphalt shingle roof on Barn #1 at the Kentucky Horse Park with a metal roof panel system. The existing asphalt shingles will be removed to deck and disposed of by the contractor. Siding on the cupolas will be removed as well. After replacing any defective sheathing, a new metal panel roof system will be installed. The cupolas will receive new siding at this time. The existing fascia will be removed and disposed of by the contractor. After replacing any defective sub-fascia, new cementitious board fascia will be installed.

The proposed Nancy Hall AML Reclamation Project, approximately 1.5 acres, consists of the construction of drainage structures along Wagon Lane, in Knott County. The site is located in the Hindman Quadrangle at 37° 16’ 4.7” N, 82° 56’ 55.5” W. To reach the site from the intersection of Hwy 15 and Rte. 160, go north on Rte. 160 approximately 4 miles to Runnels Branch Road on the left. Turn onto Runnels Branch Road and go approximately 0.25 mile to Wagon Lane. The property will be located at 133 Wagon Lane, on the left.
Reclamation activities associated with this project include the construction of 300 L.F. of combination concrete block- tied block – subdrain and 100 L.F. of combination ECB ditch-drain which will outlet into an existing drain that AML will stabilize with 145 L.F. of concrete block-tied mat. An existing CMP will be replaced with RCP and concrete headwalls. All material will be wasted onsite.
The entire project work area has been previously disturbed by coal mining operations, landowner construction, and/or road construction. These disturbances consist of significant upheaval, mixing, and removal of earthen material from deep excavation, grading, sub-surface drilling, fill material placement, and erosion. There should be no undisturbed earthen material to a depth of several feet at the areas slated for significant project-related construction activities
