accelerator and renovation of existing treatment spaces and waiting areas.

The project consists of two components: an exterior repair and an interior repair.
The exterior component consists of a partial copper roof replacement of the Barrel Roof, lower area, as indicated on the drawings. Remove portion of copper roof system, as indicated, down to existing roof substrate. Repair substrate as required, apply new underlayment and install new lead coated copper roof sheets, tying back into existing copper roof system. Anchor to substrate and solder all vertical and horizontal joints, laps and seams. Provide new PVC coated metal components; e.g. metal counter flashing, edge metal, etc.
Clean debris from roof surface and install mechanically attached new PVC roof membrane on upper flat section of metal roof, as indicated on the drawings.
The interior component includes the repair of the Barrel Vault ceiling areas and structural tubes that have been water-damaged by the roof leaks. Repair of the water damaged Gypsum Ceiling Panels, with materials matching the existing, will be completed first, followed by the application of a primer to the repaired Ceiling area as well as the Structural Tubes, followed by two finish paint coats to the entire Ceiling and Structural Tubes. The repaired Ceiling areas must also have a light sand finish aggregate surface that matches exactly the texture of the existing Ceiling.

The project is located on Old House Branch in Pike County, off KY-3419. There is a slide threatening an occupied home and an open portal at road level. Take US 23 south towards Pikeville. At the intersection of US 23 and US 119, turn left onto US 119 and travel 24 miles to Belfry. Turn right onto KY 319, travel 8 miles, and turn right onto KY 1056. Continue 3 miles and turn left on the Left Fork of Blackberry Rd for 2 miles. Turn left onto Old House Branch and travel 0.5 miles to Whitt Residence on the right side of the road.
The project area is 1.94 acres. It consists of excavation of the slide area and installation of a Type II block retaining wall. The rear deck may be removed and replaced at the engineer’s approval. Drainage control includes construction of a combination subdrain rock ditch above the wall leading to the roadside culvert. The culvert will be replaced with headwalls at inlet and outlet, and a debris barrier at the inlet.
The project also includes a portal closure at the Dorothy Hager residence several houses down. The portal is directly behind the house and will require removal/replacement of the deck, a small shed, and a section of wooden fence.
The waste area is located on the Revelation Coal site along a haul road and is approximately 0.24 acres in size.
Concrete and bituminous repair, including roadway stone, has been factored into the contract. Revegetation of the site, including agricultural limestone, fertilizer, mulch and seed will be done according to standard AML design specifications.

The project is located at 37.26715°N, 83.21389°W in Combs, KY near the AB Combs Elementary School. This project will address complaints about drainage from an OSM emergency wall (DAML LB#13515). The work site encompasses 0.97 acres of residential developed area in Combs and includes four properties where work will occur and access, and one area for staging/temporary storage. Access to the lower site work is along Peach Lane and from KY 80. The proposed upper area access is from a driveway and earthen path from North Engle St from KY 80. The entire project site (including staging area) is disturbed by residential and reclamation activities. There are no trees to clear at the site.
The project work entails installing dropbox structures and pipes to capture water and direct it to around the residences, under Peach Ln, and into the unnamed stream. This includes construction of an ECB ditch along the upper earthen path to a new dropbox structure constructed at the southern terminus of the OSM concrete wall. Water will then flow along a series of pipes with dropboxes placed at elevation changes and/or surface access points. The pipe under Peach Lane is reinforced concrete with a concrete cover to prevent settling. The outlet end of the system will connect to the creek with a standard concrete headwall. Repair fences and concrete sidewalks as needed.
The second work area entails cleaning sediment buildup from an existing drainage ditch along the north edge of the neighboring property. The sediment from the north side of the OSM wall has clogged the ditch and caused the water to flow across the residence driveway and yard. Replace the existing 24” CMP pipe with a 24” RHDPE pipe with 8” concrete cover and install standard headwalls. The current pipe is damaged and surface depressions indicate some portion of the pipe is compromised with soil/gravel piping in the bedding creating a void that will collapse a segment of the roadway.
The City of Hazard Water Department has indicated there is a 6” water main within the work limits; however, they reported the depth should be well below our excavation limits. Overhead power lines are the only other utility reported in the area. All the work area not restored in concrete will received residential seeding. We will not work within any stream (perennial or intermittent), only the banks of the existing channel. The stream at the work area has a drainage area of 0.12 square miles. The unnamed stream is a direct tributary of the North Fork of the Kentucky River near river mile 96.7. Sediment control consists of haybale barriers. Excavated materials are loaded directly into trucks and transported to the waste area eliminating onsite storage. The waste area is Harveyton waste area used by several AML projects (37º18’41”N, 83º11’ 47”W).
