A. Window Replacement (Contractor is responsible for installing one sample windows prior to ordering windows). Work includes providing all items, articles, materials, operations or methods herein listed, mentioned or scheduled on the Project Documents and I or herein, including all labor, materials, equipment, services and incidentals as necessary for their completion.
This project includes the replacement of resilient sheet flooring and integral base in two wings of the building on first floor. Work must be sequenced such that only several rooms will be replaced and completed, before proceeding with the next several rooms. Protection of the finished floor must be included until fully cured per manufactures recommendations. Dust control is required. Bedrooms and corridors to receive linoleum sheet flooring. Bathrooms to receive slip resistant vinyl sheet flooring. There is approximately 9900 sf. of new flooring being replaced, plus an additional 750 sf. being requested as an Add Alternate.
The Project consists in general of the following- Creation of dedicated locations for IT system racks and associated equipment in the Main Building in support of the Electronic Medical Records (EMR) system; extension of fiber optic circuit to each of the IT rack locations; installation of electric circuits and devices at the IT rack locations; installation of IT circuits from IT rack locations to serve wireless access points located through the building; installation of ductless split HVAC systems to serve dedicated IT rooms. General construction will include select demolition and installation of partition wall and door to create a new IDF room. Work will occur in Patient Areas and will require coordination with the Hazelwood Center.

The WKU Science Campus Phase IV is approximately 82,000 square foot new construction building that will replace the old Thompson Complex North that was raised in 2014. It will house classrooms, laboratories and offices for the Sciences per the attached specifications. The scope of work includes the relocation of approximately 11,500 lineal feet of 4-inch through 8-inch PVC water main and appurtenances, transfer twenty (20) services, six (6) connections to existing water mains (KY 448, KY 1736, Top Hill Road, KY 1638, Cedar Lane, and KY 313); ground and pavement restoration and all related work. This relocation work is required for the construction of the KY 313 Section 5 Roadway Improvements.
The main offices for Kentucky Educational Television in Lexington, KY have five courtyards that are internal to their facility. The site is located on off of KY 1086 in the community of Wayland in Floyd County, KY at coordinates N 37° 26′ 37.5”, W 82° 47′ 20.9”. The project will consist of removing existing failing gabions along Steele Creek, construction of a concrete gravity wall for stream bank stabilization, installation of a sheet piling wall and sediment removal.
