This project will replace a poorly functioning lift station and appurtenances at/near the I-64 Rest Area in Woodford County, Kentucky.
The project will construct a short length (less than 30’) of 8″ gravity sewer for the new inlet to the lift station. As shown on the plans, discovery of where this existing 8″ line bends is the Contractors responsibility. The Contractor shall provide underground power to the station as indicated from an existing disconnect. The Contractor shall construct a new discharge line as indicated on the plans including all necessary fittings and line markers. When appropriate, the Contractor shall demolish and remove the existing Lift Station. The Contractor shall install a new combination air valve in the approximate location shown on the plans. Discovery in the field of the existing force main shall be accomplished by the Contractor after consultation w/ appropriate KYTC officials (see KYTC Contacts on Sht. C02 of Plans). The Contractor shall ensure that this air valve is installed at the local high point in the force main. All surveying on the job is the Contractors responsibility.
The project shall be constructed wholly within KYTC rights of way as indicated.

The proposed project consists of a coal seam fire reclamation in Pike County. The project area is located on the United States Geological Survey (USGS) 7.5’ Pikeville quadrangle (see the attached project and site maps).
The Tim Malone Seam Fire Site consists of covering up the coal seam fire using on-site material. The site is located off Melvina Drive on Chloe Creek, near the community of Pikeville, KY. The project area is centrally located at N 37° 28’ 48.36”, W 82° 30’ 7.44”. To reach the site from the Prestonsburg Regional Field office, travel to Pikeville on US 23 South, take exit ramp onto KY-1384 toward downtown Pikeville, turn right onto KY-1426 (Lower Chloe Creek) for 0.8 miles, turn left across a concrete bridge onto Melvina Drive, last house in the hollow is Mr. Malone’s.
The Chris Johnson Seam Fire Site consists of covering up the coal seam fire using on-site material. There are exposed auger holes on an upper bench that is not on fire that will be backfilled. The site is located near the community of Pikeville, KY. The project area is centrally located at N 37° 29’ 6.9”, W 82° 30’ 8.2”. To reach the site from the Prestonsburg Regional Field office, travel to Pikeville on US 23 South, take exit ramp onto KY-1384 toward downtown Pikeville 0.6 miles, turn left onto KY-1426 for approximately 0.5 miles past shopping center, turn right for 0.1 miles to a locked gate on your left. Mr. Johnson lives in a trailer across from the gate access entrance.
Entry to the work areas will be accessed by the county road. Reclamation work consist of covering the seam fire using smothering methods to minimize the fire. When necessary water and/or foam will be pumped to quenching the burning material. There is an existing access road that will be used and maintained with roadway stone. A section of road will have to be constructed from the Chris Johnson site to the Tim Malone site. The waste material generated during project work shall be used on site in the smothering and grading portions of the project boundaries.
During the process of these construction activities, trees over 3 inches in diameter at breast height will have to be removed in the AML problem area. A sediment and erosion control program, consisting of such measures as constructing hay bales/silt fence to be maintained throughout the life of the project and prompt re-vegetation using agricultural limestone, fertilizer, seed, and mulch will be implemented on all areas disturbed by this project.

The Stephen McKinney site is located at 1402 Branhams Creek, Grethel, KY 41631 in Floyd County, KY at coordinates N 37° 28′ 40.90”, W 82° 38′ 39.60”. A portion of the bench on which the house sits has slid down toward Branhams Creek. This has made access to the other structures on the property potentially hazardous and could cause those structures to be lost if it continues to slide. Also, the material (trees, soil, debris, etc.) that is sliding could block the creek and cause flooding, including causing damage to the bridge used to access the McKinney property. The site was first inspected on 2/27/2018 and was referred to the AML design branch.
To address these concerns, the slide material will be excavated until a solid rock bench can be located. The material will be stockpiled on the bench for later use, and 120 L.F. of 21’ long W8x40 steel beams 6’ O.C. will be driven 10’ into the rock bench with sheet steel lagging between them. The soil will then be placed back behind the beams in compacted 1’ lifts with high strength uniaxial geogrid placed every 2’. Further details on this are shown in the plans. Some slide material and debris will be excavated from the area by reaching across Branhams Creek from the opposite bank. A rock silt check will be placed in the stream to mitigate issues from material entering the creek.
Additionally, several ditches and drains will be used to keep water off the slide area. A 280’ long, 1’ deep ECB ditch will be installed at the top of the hill behind the house, and two 1’ deep combination ECB ditch-drains will be installed directly behind the house (265’ long) and behind the storage structure on top of the slide (130’ long). The locations for these ditches/drains are listed on the plans. All three ditches will meet and be combined in a junction box with a steel grate which will outlet into an 18” HDPE pipe. The pipe will then outlet into an existing excavated rock ditch that runs down the hillside into the creek.
The contractor shall provide access to the site by either:
a) Installing a low water crossing (AML 50-10-7) on the downstream side of the existing bridge, or
b) Cribbing the bridge in the center and providing protective measures to the deck with steel plates or wooden runners.
If a low water crossing is chosen as the preferred method, a floodplain permit, which has already been obtained by AML, will be required. Other access to the site will be via the existing driveway and through the yard.
The waste areas for the project are located on either side of the McKinney driveway and are marked on the plans. Both waste areas may require tree removal, as may the slide area.

The site is located along 16 Mile Trace Branch Road approximately 1.1 miles from the intersection with Kentucky Highway 267 north of Hazard at coordinates N 37° 19’ 45.19”, W 83° 13’ 27.58”. The project consists of constructing 140 linear feet of 5.5’ wide Concrete Block – Tied Mat Ditch and a total of 390 linear feet of combination rock ditch / subdrain at the base of the hillslope in the backyard. The southern subdrain will discharge into a drop box and HDPE pipe to convey water to existing drainage. The northern subdrain will discharge into an HDPE pipe to convey water through a gabion wall that will be constructed in order to help stabilize a small slide that is causing a power pole to lean. Below this gabion wall a rectangular gabion ditch with 4’ flat bottom will be constructed to reach a gabion wear that will slow down the flow before water crosses under 16 Mile Trace Branch Road. A 24” DWPP pipe with concrete headwalls will be constructed realign and replace the existing pipe below 16 Mile Trace Branch Road. Material from the ditches, subdrain, and small slide area will be taken to the waste area located at N 37° 18’ 40.6”, W 83° 11’ 46.7”.

The Feds Creek Blowout HP AMLRP is located in Pike County, Kentucky at N 37° 23′
50.79” latitude and W 82° 10′ 40.41″ longitude near Fedscreek, KY. The site can be
reached from Prestonsburg, by taking US 23 south for 28.5 miles and turning right onto
US 460 east at Food City. Travel 22.5 miles and then turn onto KY 1499. Continue for
1.9 miles and turn right onto KY 366. Go 3.9 miles and turn right onto Miller’s Fork
Road. The project site begins near the last home on the right.
Project Area
A mine blowout caused water, mud and other debris to impact Miller’s Fork Creek and
roadway. The reclamation project KY DAML proposes would abate the problems by
cleaning out approximately 400’ of Miller’s Creek. Grout and rip-rap would also be
used to stabilize areas of the creek bank impacted by the blowout. Two 60” culverts
would also be replaced with RHDPE pipe. Both of those culverts would also have inlet
and outlet headwalls. At the outlet end of the western most headwall a 20’long by
10’tall Sheet Pile Wall would be constructed to help protect and stabilize the hillside.
An 18” HDPE pipe, with inlet headwall, would also be installed to facilitate existing
drainage impacted between two homes on Miller’s Fork Road. The roadside ditch
would also be cleaned out as needed.
Silt fence and/or bales and rock check dams are proposed for proper silt control. A
utility relocation bid item is included, which would replace “in kind” qualifying features
as disturbed to facilitate the abatement plan. Likewise, roadway stone and concrete
bid items are included, which would repair “in kind” any damage to the driveways off of
Miller’s Fork Road.
Waste Area
N 37° 24′ 2.52” latitude and W 82° 14′ 24.29″ longitude.
Waste material would be end dumped and then lightly graded with a bulldozer, or other
heavy equipment as directed by the engineer.
General
All disturbed areas (site and waste area) would be re-revegetated according to KY
AML standards. Trees greater than 3” at breast height could be removed to facilitate reclamation activities.

The project involves the complete removal of the existing HVAC system. This will include removal of the existing hot water boiler system, expansion tanks, housekeep pads, circ. Pumps, air separator and controls.

Piping also, a new rooftop HVAC unit to air condition the Drill Hall. This contractor will need to make all of the interlocks between equipment for controls. Complete the installation of the new windows and doors so security for the building is not breeched.
If the add alternate #1 is accepted install polished concrete floors in all areas that now have VCT floor covering.
