The Randy Gibson Slide Phase II HP AML Reclamation Project is located in Knott County, in the Pine Top community. GPS coordinates: 37* 16′ 43.75″N -82* 49′ 57.0″W. From the Hazard AML office. Travel HWY 15 southeast for 16.3 miles. Turn left onto Hwy 160 and travel approximately 3.3 miles. Turn right onto Hwy 582 travel 8.1 miles to residence on right of Hwy turn right across bridge bare to the right travel approximately 400 foot parallel to road to residence.
Slide (Site Area)
The slide is directly behind and to the right of the Gibson house in a drain. AML proposes to remove slide material and transport to designated waste areas. Construct a Temporary low water crossing for transporting material to waste areas. Install 24″ HDPE pipe with headwalls to provide drainage from slide across yard of residence. Existing Concrete Debris Barrier Wall is to be remove upon completion of project. An items of directional drilling to relieve pressure from underground mining and limestone sand to treat water. Windrow trees and regrade slide material on site. Roadway stone is included to provide access to the slide area. Sediment control will be provided by installation of silt fence or bales as directed. Utility Relocation/Replacement is included to replace any in kind item disturbed.
No trees 3″ dia or greater will be removed from the area.

There are 3 designed waste areas along Hwy 582 above and below site, see waste area planview.

Louisville Water Company is requesting Bids for the supply and installation of approximately 2,930 LF of 24” ductile iron water
main, including all appurtenances, located in Shepherdsville, KY, starting at a tie-in point located at Preston Hwy (Buckman Str) at
Hwy 44, heading west on Hwy 44, then south on Plum Str, then east on Joe B. Hall (2nd Str) across Preston Hwy (S. Buckman Str) to
the tie-in point at N. Walnut Str. Contractor shall supply and install 1,985 +/- LF of 24” class 250 restrained joint water main and 945
+/- LF of 24″ class 250 water main and all other materials. Included are 2 bore and jacks; 1.) 40 LF of 42” steel casing across Hwy 44
at Plum Str. and 2.) 90 LF of 42” steel casing across Preston Hwy. at Joe B. Hall Ave, 5 drains, 5 air valves, install roadside ditch with
new drive culverts, storm pipe, driveway restoration, yard restoration, roadway restoration / milling and paving, 35 +/- LF of 8” water
main relocation, 2 ~ 24” tie-ins and site restoration. Contractor is responsible to verify all utilities affecting construction, including all
utility crossings, prior laying pipe. Contractor should note that all profile information is assumed and adjustments to depth of the water

The Shonia Bentley Slide is located at 221 Official Hollow in the community of David, KY. The coordinates for the project site are 37°35’50.41″N, 82°53’12.864″W.
The project will consist of constructing an access through the yard of neighbor Larry Tussy located at 193 Official Hollow, removing slide material which has impacted that residence from the Bentley property above, installing 64 linear feet of Type II Non-Reinforced Concrete Block Wall with safety fence on a concrete footer. The wall will have wing walls up to 10 feet long on each end as directed by the Engineer. A sub drain will be installed behind the wall and routed to an existing drop box that inlets into an existing culvert. A new drop box inlet will be poured in place to accommodate the added drain and a new grate installed for safety. The wall will be backfilled with No. 2 sized crushed limestone to the top and Class II sized backfill will be placed on the slope above to support the Bentley yard and home above and stabilize the slope. There are three large trees and several small saplings that lie on the unstable slide area and will be removed for the safety of the residents as well as access and progression of work. The construction entrance will be removed upon completion and all impacted areas will be revegetated under residential seeding standards. The inlet of the culvert in which the wall drainage is to be routed is occluded with debris and will be cleaned out to allow open flow as will a portion of the ditch feeding it which is currently lined with Concrete Block Tied Mat. This work will fall under Site Preparation and will be directed to be done as soon as the contractor mobilizes.
The slide debris will be hauled by truck to a waste area located approximately 2.9 miles away. The coordinates for the waste area are 37°35’19.493″N, 82°52’1.319″W. Work will be done in accordance with the plans, waste area permit, and direction from the ENGINEER. All material will be graded and seeded as per residential seeding seed mix.

The Johnny Stewart Slide AML Reclamation Project includes approximately 4.4 acres of
total disturbance, and will consist of the reclamation of a double-headed landslide, and the
collection and control of underground mine drainage that is affecting at least three residences on
Upper Branch Road, near Elkhorn City, Kentucky. One portion of the landslide, which is
threatening to block Upper Branch Road and is undercutting a barn on the slope above, will be
reclaimed by means of the construction of approximately 114 linear feet of 15 foot tall gabion
basket retaining wall. The other portion of the landslide, which is threatening to block the
residential access drive to the Johnny Stewart residence will be reclaimed by construction of
approximately 92 linear feet of 12 foot tall modular block retaining wall. Subdrain will be
installed on the inslope side of the bench where the barn, which is being undercut by the first
portion of the slide that is described above, is located. The downslope portion of a drainage
system constructed under an OSM AML Emergency reclamation project, which is carrying water
collected from a number of abandoned underground mine entries and adits located well above
the landslide, will be reconstructed. It is the water that is escaping from this drainage system that
is providing the lubrication for the landslide activity being addressed at the lower level. Other
reclamation action includes the construction of a combination ditch/subdrain along the upslope
edge of the Johnny Stewart residential yard, and the improvement of an existing ditch that carries
water off of the house seat. On the slope directly overlooking the Stewart residence and a
downstream portion of Upper Branch Road, seven collapsed and draining abandoned mine
portals will be reopened, drainage structures will be installed to relieve any potential buildup of
water pressure within the mine workings, and the mine openings will then be resealed. As there
is no instability resulting from the drainage from these openings, water from these portals will be
allowed to maintain its current courses down the hillside.
Earthen material excavated during the construction of this project will be placed in a fill
area located almost directly on the opposite side of the valley floor from the project site. If this
area becomes filled, there are three areas within the project site (located on mine benches above
the landslide area) where additional material will be stored. Subdrain is included to prevent
drainage from crating a lubrication plane beneath fill placed in these supplemental areas.
Revegetation of residential lawn areas, including the primary excess earth fill area, will
be accomplished by means of the Standard AML Residential Seed Mixture. All other areas will
be revegetated utilizing the Standard AML Reclamation Seed Mixture. Trees will have to be
removed from the primary excess earth fill area, as well as from the landslide area. Other than
the reopening and permanent closure of the seven abandoned underground mine openings (as
described above), there will be no disturbance of any cave-like habitats. The only direct
disturbance of any streams will be the installation, maintenance, and removal of a rock sediment
check in Upper Branch immediately downstream of the landslide reclamation site.
